The Leeton Local Health Advisory Committee Collaborative Care Project for Leeton - 2025 Reflection and 2026 Outlook has provided a deep understanding of the local health system and strengthened connections with the community, local providers and partner health agencies.

Over the past 12 months, the project has delivered:

• Strong community insights

• Valuable input from local health providers

• Service and program mapping

• Connections with subject matter experts

• Program updates and coordination

• The establishment of our top three health service priority areas.

In 2026, the Collaborative Care Project will move into implementation, delivering projects aligned to these priorities with a focus on practical, sustainable improvements to access and care.
